Akobi's life was rescued by stormtrooper TK-622 while stationed on Malastare in 2 BBY, forging a friendship between the two.
While Commander Akobi was combating a Rebel uprising on his homeworld in 0 BBY, he mistakenly gave the order to bomb a medical facility located in Ettam, a town situated on Ralltiir. This was partly caused by Rebel interference with Imperial communications, compounded by Akobi's faulty memory of the terrain. Despite the incident, the Empire concealed the truth and lauded him as a hero. Shortly before his division's redeployment, Akobi narrowly escaped death from a Rebel-planted bomb, once again thanks to 622's intervention.
Following the conclusion of his division's assignment on Ralltiir, Akobi was reassigned to the first Death Star, where he faced an attack from a RA-7 droid. TK-622's swift response was crucial in saving his life. However, the Rebel saboteur Krono Relt persisted in his attempts. En route to an awards ceremony, Akobi was assaulted by a malfunctioning Viper probe droid, resulting in severe electrocution of the Imperial officer.
Akobi was admitted to a medical center, where his chances of survival beyond a week were deemed slim. In his final moments, he confessed to his sole confidant, TK-622, the truth regarding the atrocities committed on Ralltiir. His physical remains were obliterated when the Death Star's primary reactor reached critical mass.
