Pattern maker Brian Archer constructed the "reveal" helmet for Darth Vader at Elstree Studios for the movie Star Wars: Episode VI Return of the Jedi. His contribution as a Supervising Model Maker during the Elstree Studios production was not officially recognized in the credits.
