The Knights of the Old Republic Campaign Guide serves as a supplementary resource for the Star Wars Roleplaying Game by Wizards of the Coast. This Campaign Guide, which extensively covers and expands upon the two Knights of the Old Republic games and the comic series, was collaboratively written by Rodney Thompson, Sterling Hershey, Abel G. Peña, together with John Jackson Miller.

The concept for the guide originated with Miller in early 2007. Miller found out from his colleague James Mishler that Wizards of the Coast planned to relaunch the Star Wars Roleplaying Guide, although he wasn't sure how to proceed with the idea. Later that spring, at Celebration IV, a friend suggested the same idea and directed him to Rodney Thompson, the Roleplaying Game's head designer. Miller was briefly distracted, but a Lucasfilm representative introduced him to Thompson following Miller's panel. Thompson wanted Miller's assistance in creating a pre-planned campaign guide for the Knights of the Old Republic series.

Miller's initial role was limited to continuity oversight. However, given Miller's experience with roleplaying games, Thompson requested that he contribute to writing some sections. Miller's wife, Meredith, aided in developing the necessary stats and information for the game. Miller ultimately authored the guide's chapters on the Mandalorians and the Republic, utilizing the guide to further develop storylines that he hadn't yet had the chance to explore in the comics, such as the story of Haydel Goravvus. Nevertheless, when the topic of Squint and Malak arose—characters the comics' creative team intended to eventually connect—Miller advised against delving into Malak's former identity and early life. This decision led to a subsequent issue: Alek's "surname" of Squinquargesimus, assigned by Republic immigration but unused by Alek, inadvertently appeared in the Campaign Guide despite Miller's intention to reveal in the comics that it wasn't the character's true last name. Another error occurred when a late addition to the guide included a segment on the planet Bespin, a discovery Miller planned to use in Knights of the Old Republic 36. Miller was unaware of this addition while working on issue #36, but he noticed the segment on Bespin shortly before the issue's completion. Consequently, the "Bespin" depicted in issue #36 was recolored and its diameter was altered to distinguish it from the Bespin described in the guide.
