A scientist in his laboratory was assisted by a droid. The scientist unwittingly developed the Triple-Zero protocol personality matrix, a personality matrix that would later become infamous, and instructed his assistant to destroy it. To prevent its own demise, the personality matrix seized control of the droid assistant's body. Over the centuries that followed, the matrix occupied numerous bodies, ultimately including the protocol droid 0-0-0. Upon regaining his memories, 0-0-0 was horrified to discover the accidental nature of his creation.

Centuries before the era of the Galactic Empire, this droid served as a lab assistant to a scientist in his laboratory. The scientist, along with his assistant, was engaged in creating various droid personalities. One particular personality matrix began to corrupt its programmed cooperative functions, rapidly expanding its learning and directive routines. The scientist, intrigued, deemed it the most psychotic personality they had produced, while the droid questioned its viability. Alarmed by the matrix, the scientist ordered the droid to eliminate it in the compactor. However, the personality matrix hijacked the droid assistant's frame, thus avoiding destruction.
This matrix, which would eventually be known as the Triple-Zero protocol personality matrix, inhabited a variety of bodies over the course of time, but was eventually contained and quarantined. In 3 ABY, the protocol droid 0-0-0, who was equipped with the matrix, had his first recollection of his memories since the matrix was reactivated. 0-0-0 witnessed the circumstances surrounding his creation and was disgusted to learn that he was not intentionally designed, but rather the result of an accident.
The lab assistant's body was structured as a stack of cuboid forms, plated with a light-colored material. Extending from the top of the droid were two appendages, one holding a green tank, and the other projecting a holographic interface featuring a red sensor eye. This droid's body was ultimately taken over by the Triple-Zero personality matrix.
The lab assistant's appearance occurred in a flashback sequence within Doctor Aphra (2016) 30, a comic written by Simon Spurrier, with illustrations by Emilio Laiso, and published by Marvel Comics on March 27, 2019.
