Uthar Wynn, who was the Headmaster of the Sith Academy on Korriban and a Sith Lord, created a double-bladed lightsaber during his time as an apprentice serving under Jorak Uln.
Wynn wielded this lightsaber in 3956 BBY during a duel within the tomb of Naga Sadow. His opponent was Yuthura Ban, his own apprentice, who conspired with the amnesiac Revan, formerly the Dark Lord of the Sith, to assassinate him. He used this weapon in a final, failed attempt to slay those who had betrayed him, resulting in his death. It is assumed the lightsaber remained with his body.
