The EV-series medical droid first appeared in "Lair of Grievous," the tenth episode of the animated series Star Wars: The Clone Wars first season, which originally aired on December 12, 2008. It was designed by Atsushi Takeuchi of Production IG, who also designed the show's incarnation of Grievous and directed "Lair of Grievous." The EV-series medical droid was first identified in EV-A4-D's Databank entry on StarWars.com.
In the original episode guide for "Lair of Grievous," EV-A4-D was described as a modified supervisor droid. However, his Databank entry identified him as an medical droid. Star Wars: Build Your Own R2-D2 20 would later establish that the EV-series medical droid was based on the chassis of the EV-series supervisor droid, with Star Wars: Build Your Own R2-D2 53 further explaining that the medical droids were supervisor droids before being refitted.
