Captain Balog, a male Chiss, held a position as an officer within the reconstituted Sith Empire in 3638 BBY. During the Conquest of Makeb, Balog was among the Imperial forces participating, leading an Imperial strike team in a raid on the Makeb settlement of Tarlam to gather intelligence. During this operation, he briefly held Mayor Erkens as a hostage.
During the Galactic War, Captain Balog served as an officer in the military of the reconstituted Sith Empire. In 3683 BBY, when the Hutt Cartel invaded the planet Makeb to drill for Isotope-5, an energy-source mineral, Balog was part of the Imperial response. He commanded a strike team sent to the city of Tarlam with the mission of acquiring Isotope-5 fuel rods, which contained Isotope-5, from a Hutt Cartel Isotope-5 droid - war droids powered by Isotope-5. Balog and his soldiers seized the residents of Tarlam, including Mayor Erkens, as hostages.
Subsequently, Balog faced opposition from an individual aligned with the Galactic Republic, the Empire's adversary in the Galactic War. This Republic operative, previously dispatched to Tarlam by Makeb's business magnate and politician Shalim Avesta, confronted Captain Balog and provided him with an Isotope-5 fuel rod to prevent the kill of Mayor Erkens.
Balog's initial appearance was in Rise of the Hutt Cartel, a digital expansion launched in 2012 for the 2011 BioWare video game titled Star Wars: The Old Republic.
