The clash known as the Battle of Khorm took place during the period of the Clone Wars. It pitted the forces of the Galactic Republic against those of the Confederacy of Independent Systems at some point in the timeframe spanning from 22 BBY to 21 BBY. The conflict was waged over the control of the natural resources found on Khorm. Leading the Republic forces were Jedi General Anakin Skywalker and Clone Commander Wolffe, who commanded their troops against the Confederate troops under the leadership of Commander Asajj Ventress, a dark side assassin. The two sides engaged in combat on Khorm.
Amidst the heat of battle, Ventress and Wolffe found themselves in a fierce confrontation. This encounter resulted in Wolffe losing his right eye and acquiring a prominent scar on his face. Despite these injuries, his exceptional fighting prowess allowed him to survive. Wolffe, bearing the scars of this battle with pride, continued his service to the Republic, replacing his lost eye with a cybernetic implant.

The first, though not direct, mention of the Battle of Khorm within the new Star Wars canon came in the Encyclopedia entry of Clone Commander Wolffe on StarWars.com. The entry included a biography gallery, one slide of which depicted a scene from the Star Wars: The Clone Wars television series episode "Holocron Heist" from the second season. The caption accompanying the image explained that Wolffe's missing eye, as seen in the episode, was lost "on the battlefield." The name of that specific battle was later revealed to be the Battle of Khorm in the 2015 reference book Ultimate Star Wars.
Henry Gilroy, a writer for Star Wars: The Clone Wars, once considered developing episodes of the series around the Battle of Khorm. However, the idea was ultimately abandoned due to budgetary limitations affecting the show's production.

Gilroy's concepts for the battle were instead brought to life within the Star Wars Legends continuity. The Battle of Khorm was depicted in a 2009 story arc within the Star Wars: The Clone Wars monthly comic series, titled In Service of the Republic. This arc was written by Gilroy and Steven Melching, with illustrations by Scott Hepburn. In this comic series version, the battle revolved around the control of the planet's valuable agrocite mineral and concluded with a Galactic Republic victory. The comic also showed Asajj Ventress inflicting the eye injury upon Wolffe.
However, as the The Clone Wars comics became part of the non-canon Legends timeline, subsequent canon sources presented different details about the Battle of Khorm. Notably, the Star Wars: Force Collection digital card game established that Anakin Skywalker participated in the battle, an event not depicted in the comic series.
